Know your schedule. What day and time works best for you and if you were referred to a specific provider or location.
Please have insurance information available. We will ask for your name, birthdate, contact information and insurance policy.
You will receive intake forms via text or email. You can also print from our website. If unable to complete before, please arrive 20 minutes prior to your appointment.
A confirmation text will be sent 1 day and 1 hour prior to your appointment. Please confirm!
Upon arrival:
Sign in at the front desk.
Provide ID and insurance card.
Confirm intake forms were received if completed prior to your appointment.
Before meeting with your provider, a Health Care Assistant will perform vitals and Range of Motion (if necessary).
